Scientists Detect Alzheimer’s Warning Signs a Decade Before Symptoms Appear

Groundbreaking blood tests and brain scans may soon allow doctors to identify people at high risk of Alzheimer’s years before memory loss begins, raising hopes for earlier treatment and prevention.

Researchers have unveiled major advances in the early detection of Alzheimer’s disease, with new studies suggesting that subtle biological warning signs can now be identified up to a decade before patients develop clear symptoms of cognitive decline. The findings, published in The Lancet, are fueling optimism about a future in which routine blood tests and advanced imaging scans could transform how the world’s most common neurodegenerative disease is diagnosed and treated.

The first study focused on apparently healthy middle-aged adults between the ages of 56 and 69 who had been monitored for more than 35 years. Scientists from the United States analyzed blood samples from 1,350 participants using newly approved tests designed to detect abnormal accumulations of beta-amyloid and tau proteins, the two biological hallmarks most strongly associated with Alzheimer’s disease.

Researchers discovered that roughly 6% of participants already showed molecular signs linked to Alzheimer’s despite displaying no obvious dementia symptoms. Those individuals were also found to have subtle difficulties involving verbal memory and cognitive processing speed, suggesting that neurological changes may begin many years before traditional diagnosis becomes possible.

The study found that individuals carrying certain biomarkers faced dramatically higher risks of future cognitive problems. One marker increased the likelihood of verbal memory difficulties more than fourfold, while another was closely associated with slower mental processing. Scientists emphasized, however, that the presence of these proteins does not guarantee a person will develop dementia, and many uncertainties remain regarding how accurately these markers can predict future disease progression.

The research was led by neurologist Kristine Yaffe of the University of California, San Francisco, whose team argues that earlier identification could allow doctors to intervene during middle age through lifestyle changes such as exercise, healthier diets, and improved cardiovascular care. Researchers also pointed to the potential role of new Alzheimer’s drugs, including lecanemab and donanemab, which have shown modest success in slowing disease progression during early stages of the illness.

A second study published simultaneously in The Lancet explored a different method for early diagnosis using advanced brain imaging. Researchers in the United States and Canada tested an experimental imaging tracer called MK6240 on nearly 700 participants. The technology, developed by the American company Lantheus, enabled doctors to detect tau protein accumulation earlier than existing diagnostic methods in cognitively healthy adults aged between 50 and 89.

Current diagnostic tools for Alzheimer’s often rely on expensive brain scans or invasive spinal fluid extraction procedures. Blood tests could offer a simpler and more accessible alternative, potentially opening the door to widespread screening programs in the future. Experts say this could become increasingly important as new therapies are designed to work only during the earliest phases of the disease.

Alzheimer’s currently affects an estimated 50 million people worldwide, yet specialists warn that more than half of mild cases remain undiagnosed. In many countries, patients wait between two and three years after the first symptoms appear before receiving an official diagnosis. During that period, opportunities for effective early intervention are often lost.

Despite the scientific excitement, many experts remain cautious about introducing mass screening programs for healthy individuals. Researchers from Finland’s Institute for Health and Welfare warned that these biomarkers become less reliable in younger populations and could generate large numbers of false positives, potentially overwhelming healthcare systems and causing unnecessary anxiety among patients.

Neurologists also stressed that existing drugs do not cure Alzheimer’s but merely slow its progression. Because of this, several specialists argue that widespread testing of healthy people is not yet justified outside research settings. Pascual Sánchez Juan, scientific director of Spain’s CIEN Foundation, noted that while the studies are highly valuable scientifically, current treatments still cannot reverse the course of the disease.

Other experts believe the field may be approaching a turning point. Juan Fortea, a neurologist at Hospital Sant Pau in Barcelona and co-author of the imaging study, said modern neurology is entering a new era shaped by blood-based diagnostics and emerging therapies. He predicts that if ongoing trials demonstrate clear benefits from treating patients before symptoms emerge, population-wide Alzheimer’s screening could eventually become as routine as cholesterol testing or cancer screening programs.

The growing availability of private biomarker testing has already sparked ethical concerns. Specialists warn that healthy individuals are increasingly requesting these tests despite the limited predictive certainty they currently offer. Experts fear the rapid commercialization of early diagnostic tools could outpace scientific understanding, leaving patients with uncertain results and few proven treatment options.

Even so, the latest discoveries are widely viewed as one of the most significant breakthroughs in Alzheimer’s research in years, offering new hope that the disease may eventually be detected — and possibly slowed — long before devastating symptoms take hold.
