Global Breast Cancer Rates Surge Amid Low Awareness of Breast Density Risks

The rising global burden of breast cancer, coupled with low awareness of breast density risks, underscores the urgent need for comprehensive education and equitable access to advanced screening methods.

Breast cancer remains the most prevalent cancer among women worldwide, with the World Health Organization (WHO) projecting a significant increase in cases and deaths by 2050. A recent analysis by the International Agency for Research on Cancer (IARC) estimates that by mid-century, annual new breast cancer cases will reach 3.2 million, with 1.1 million deaths per year.

A critical yet underrecognized factor contributing to breast cancer risk is breast tissue density. Dense breast tissue, characterized by a higher proportion of fibrous and glandular tissue compared to fatty tissue, not only increases the risk of developing breast cancer but also complicates its detection through standard mammography. On mammograms, both dense tissue and tumors appear white, making it challenging to distinguish between the two.

Despite its significance, awareness of breast density as a risk factor remains low globally. A multinational survey conducted in 2024 across six countries, including the United States, Germany, Italy, South Korea, Brazil, and Mexico, revealed that only 39% of women were familiar with the concept of high breast density in the context of breast cancer, and merely 31% recognized it as a risk factor.

In response to this knowledge gap, the U.S. Food and Drug Administration (FDA) implemented a regulation in September 2024 mandating that mammography facilities inform patients about their breast density. The regulation requires that mammogram reports include specific language explaining how breast density can influence the accuracy of a mammogram and recommend that patients with dense breasts discuss potential risks and additional screening options with their healthcare providers.

Breast density is categorized into four levels: A (almost entirely fatty), B (scattered areas of fibroglandular density), C (heterogeneously dense), and D (extremely dense). Categories C and D are considered “dense” and are associated with higher cancer risks. Approximately 40% of women aged 40 and older fall into these dense categories.

For women with dense breast tissue, additional imaging tests such as ultrasounds or magnetic resonance imaging (MRI) may be recommended to improve cancer detection. However, access to these supplementary screenings varies globally, often limited by healthcare infrastructure and insurance coverage.

Healthcare systems worldwide must prioritize informing women about breast density and its implications to enhance early detection and improve outcomes in the fight against breast cancer.
